New Patient Registration
If you are new to the area and would like to register with one of our GP’s please ask at our reception. If you have your medical card then please bring this along with you.
Following your registration you will be required to have a new patient check to enable your registration to be fully complete. An appointment will be offered with our practice nurse.
Changes to Personal Details
Please make sure that you keep us up-to-date with your contact details. Please inform any of the staff members if you need to change any of the details we have for you, i.e. - name change, address change or telephone number. In the instance of a name change we may need to ask for proof, i.e. marriage certificate, deed poll, driving licence or passport. This will be photocopied and scanned onto your medical records and a copy will also be sent to our medical records department for them to amend their records.
